Do you want to be eligible to win a gift card and take care of your health at the same time?
If you participate in Ƶ’s group insurance plan administered by Blue Cross Blue Shield, you have a chance to win a variety of prizes while you get healthy. It only takes 10 to 15 minutes to take a health assessment for the BCBS Healthy Outcomes program. If you complete the health assessment by March 31, you will be entered into a drawing for a $25 gift card Fifty cards will be given out.
That’s right – in the time it takes to go get a cup of coffee, you could and get started on a wellness program tailored just for you. Simply answer the questions and quickly receive a snapshot of your current health, along with a plan for moving forward.
The assessment will:
- Evaluate the current state of your health;
- Identify health risks you might not be aware of;
- Offer recommendations such as programs for stress relief, fitness or better nutrition; and
- Be used to personalize your Healthy Outcomes homepage.
